Prime Care Home ensures that all hospital transfers are handled quickly, safely, and with full coordination to protect the life, health, and dignity of every resident.

Hospital transfer is arranged immediately when a resident shows signs of a medical emergency or when advised by an on-call doctor or nurse.

Staff remain calm and provide first aid support while waiting for medical assistance.

An ambulance is contacted without delay when required.

The resident is safely prepared and escorted during transfer.

Prime Care Home will:

Notify the resident’s family or authorized representative immediately.

Provide complete information about the resident’s condition and destination hospital.

Ensure all relevant documents, medication lists, and care information are ready for hospital staff.

Responsibilities:

All hospital and medical expenses are the responsibility of the family.

Prime Care Home assists with coordination but does not provide medical treatment.

Staff stay focused on resident safety and comfort during the entire process.

Documentation:

Every emergency transfer is recorded in the Incident and Emergency Log.

Details include time, reason for transfer, actions taken, and persons notified.
This policy ensures that emergency situations are managed professionally, efficiently, and with compassion, giving residents immediate access to proper medical care while keeping families fully informed.
