Prime Care Home is committed to ensuring that every resident admitted to our facility receives care that is safe, appropriate, and within our non-medical scope of services. Admission is based on a thorough assessment to confirm that the resident’s needs can be properly supported.

Eligibility for Admission

Prime Care Home accepts elderly individuals who require non-medical assisted living care.

Residents must be medically stable and not require continuous hospital or intensive medical treatment.

Applicants whose needs exceed non-medical care may be referred to a hospital or specialized facility.

Assessment Process

Each applicant undergoes a personal assessment before admission.

The purpose of the assessment is to determine if Prime Care Home can safely and properly meet the resident’s needs.

The care plan is reviewed and updated regularly to reflect any changes in the resident’s condition.

Approval of Admission

Admission is confirmed only after full review and approval by Prime Care Home management.

Prime Care Home reserves the right to decline admission if the resident’s needs are beyond our non-medical care capacity.

Provide complete and accurate medical and personal information.

Supply prescribed medications and necessary medical supplies.

Inform Prime Care Home immediately of any change in health condition.

Follow all policies, payment terms, and visiting rules.

Where is the assessment done?
The assessment is conducted at:

Prime Care Home (preferred), or At the resident’s current home or hospital (if needed and arranged in advance).

How many days does the assessment take? Initial assessment: 1 day (interview, observation, and document review).

Final evaluation and care plan preparation: 1–2 working days.
-
Total process: usually within 2–3 working days.

Who conducts the assessment?
The assessment is carried out by: The Care Manager / Senior Carer, and Reviewed and approved by Prime Care Home Management.
If needed, input may also be taken from:

On-call Nurse or Doctor (for medical clarification only, not diagnosis).
